GLP-1 medications such as semaglutide (Ozempic, Wegovy) and tirzepatide (Mounjaro, Zepbound) make it much easier to eat less. That leads to real weight loss, but not all of the weight lost is fat. Without a plan, a meaningful share can be muscle, and muscle is what keeps you strong, mobile and burning energy.
How much of the weight is muscle?
In body-composition studies within the large semaglutide trials, lean mass (which includes muscle) made up roughly a quarter to a third or more of the total weight lost. Some lean-mass loss happens with any large weight loss, including dieting and surgery. But because GLP-1s can reduce appetite so strongly, it is easy to eat too little protein and lose more muscle than you need to.

- Metabolism: muscle uses more energy at rest than fat, which helps with keeping weight off later.
- Strength and balance: especially important as we age, to protect independence and reduce the risk of falls.
- Blood sugar: muscle is the body's biggest user of glucose.

Five habits that protect your muscle
- Eat protein first, at every meal. When appetite is small, start the meal with the protein: eggs, Greek yogurt, cottage cheese, fish, chicken, tofu, lentils or a protein shake. Many experts suggest around 1.2 to 1.6 grams of protein per kilogram of body weight per day during weight loss for most healthy adults. Ask your doctor or dietitian what is right for you, especially if you have kidney disease.
- Strength-train at least twice a week. Resistance exercise is the strongest signal to keep muscle. Bodyweight squats, push-ups against a counter, resistance bands or weights all count. Work the major muscle groups and gradually increase the challenge.
- Keep walking. Daily walking supports heart health, blood sugar and energy without adding much hunger.
- Don't under-eat. Very low intakes speed up muscle loss. If you are eating very little, ask your prescriber about the pace of your weight loss and your dose.
- Cover your nutrients. Smaller meals can mean fewer vitamins and minerals. Choose nutrient-dense foods, drink water through the day, and ask your care team whether a multivitamin or mineral supplement makes sense.
Track more than the scale
The scale can't tell muscle from fat. Watch how your clothes fit, track your waist measurement, and notice your strength: if you can lift, climb stairs and carry groceries as well as before, you are likely protecting your muscle.
